Our Privacy Commitment To You

At Brockton Physiotherapy, we are committed to protecting your privacy and ensuring the confidentiality of your personal health information (PHI). We follow the Personal Health Information Protection Act (PHIPA) and the standards set by the College of Physiotherapists of Ontario.

This notice explains what information we collect, why we collect it, how we use it, how we protect it, and what your rights are.

What Information We Collect

We collect only the information required to provide safe, effective health care, including:

  • Your name, date of birth, contact information

  • Your health history and family health history

  • Assessment findings, treatment records, and clinical notes

  • OHIP number, insurance and billing information

  • Information about your functional status, lifestyle, or relevant personal context

Why We Collect, Use, and Share Your Information

We collect, use, and disclose your information for the following purposes:

Primary Purposes

  • To assess, plan, and provide physiotherapy and related services

  • To communicate with other health-care providers involved in your care (“circle of care”)

  • To track your progress, outcomes, and continuity of care

Related Purposes

  • To obtain payment (from you, insurers, WSIB, legal representatives)

  • To support quality improvement, safety, and training

  • To comply with regulatory requirements (e.g., College audits)

  • To respond to legal or statutory obligations

We will only use or disclose your information for reasons not listed above with your express consent, unless we are legally required to do so.

Your Consent

By receiving care at Brockton Physiotherapy, you provide implied consent for collection, use, and sharing of information within the circle of care.

We require express consent to:

  • Send information to anyone outside the circle of care

  • Use your information for newsletters, marketing, special events

  • Share information with employers, insurers (outside billing), lawyers, or third parties

You may withdraw consent at any time.

Your Rights

You have the right to:

  • Access your personal health record

  • Request corrections

  • Place a restriction (known as a “lock box”) on specific information

  • Ask questions about how your information is used

  • Make a complaint to us or to the Information and Privacy Commissioner of Ontario (IPC)

We will respond to access requests within 30 days, as required by PHIPA.

How We Protect Your Information

To keep your information safe, we use:

  • Locked storage for paper records

  • Secure passwords and encrypted systems

  • Limited staff access based on job role

  • Confidentiality agreements with all staff and service providers

  • Secure transfer procedures for email, fax, and electronic records

  • Staff training on privacy and PHIPA compliance

We retain your record for 10 years from your last visit, or 10 years after a minor turns 18, and then securely destroy it.

If There Is a Privacy Breach

If your information is lost, stolen, or accessed without authorization, we will:

  • Notify you as soon as possible

  • Provide information about what happened

  • Explain what steps we are taking to correct the issue

  • Provide your right to complain to the IPC
